Which forms you need depends on your plans for Winter Camp.
I'm just doing Winter Camp at Home
This option means you aren't going to meet anyone outside your family and will only be online for Winter Camp.
I'm a youth attending Winter Camp At Camp
These are the forms you need if you're a youth who plans to meet some of us for Winter Camp At Camp.
- Registration Form
- Medical Form - We only need parts A & B.
- Youth Covid Waiver
- Supplemental Medical Waiver - Required if you are in a vulnerable class
I'm 18 or older and attending Winter Camp At Camp
- Copy of a current Youth Protection Training certificate
I'm an adult attending Winter Camp At Camp
These are the forms you need if you're a youth who plans to meet some of us for Winter Camp either locally or at camp.
- Registration Form
- Medical Form - We only need parts A & B.
- Adult Covid Waiver
- Supplemental Medical Waiver - Required if you are in a vulnerable class or over 60 years old.
